Employment Standards
Employment Standards Claim [ Form #: MOL-ES-0002 ]
This form is used to recover money and benefits from an employer.
Hours of Work and Averaging Hours [ MOL-ES-001 ]
This form is used by employers if they want an employee to work more than 48 hours a week or they want to average an employee's hours of work for overtime pay purposes.
Health and Safety
Notice of Project [ Form 0175 ]
This form is used by constructors to notify the Ministry of Labour of construction projects as required by section 6 of the Regulation for Construction Projects (O. Reg. 213/91).
Registration of Constructors and Employers Engaged in Construction [ Form: 1000 ]
This form is used by every constructor and employer engaged in construction work to register, as required by section 5 of the Regulation for Construction Projects (O. Reg. 213/91), before work begins on a construction project.
Window Cleaning / Asbestos Removal / Trench Work Notification
This form is used by employers to notify the Ministry of Labour of window cleaning operations, asbestos removal operations, and trench work where a worker may enter a trench which is more than 1.2 metres (4 feet) deep.
Labour Relations
Decision of Arbitrator or Arbitration Board under Section 48 of Act [ Form #6 ]
This form is used when filing an arbitration decision with the Superior Court of Justice.
Declaration by Employer Bargaining Agency or Employers’ Organization [ Form #7 ]
This form is used when filing a declaration with respect to lockout votes with the Minister of Labour.
Declaration by Employee Bargaining Agency or Affiliated Bargaining Agent [ Form #8 ]
This form is used when filing a declaration with respect to strike or ratification votes with the Minister of Labour.
Decision of Arbitration Board under Section 103 of Act [ Form #9 ]
This form is used when filing an arbitration decision dealing with damages for an unlawful strike or lockout with the Superior Court of Justice.
Ontario-Quebec Construction Labour Mobility (Jobs Protection Office)
Contractor Registration Application for Specialized Work [ Form: #JPO-001 ]
This form is used by contractors who want to bring their workers into Quebec to do specialized construction work. This type of work is usually associated with the provision of a warranty. Contractors who work along with their workers must also fill out a Worker Registration.
Worker Registration Application: Specialized Work [ Form:#JPO-002 ]
This form is used by workers who want to work in Quebec with their Ontario contractor to do specialized construction work. This type of work is usually associated with the provision of a warranty.
Contractor Registration Application
Trade Activities [ Form: #JPO-003 ]
This form is used by contractors who want to bring their qualified but uncertified workers to do work in Quebec. These workers normally have experience in certain aspects of a voluntary trade but do not hold a certificate for that trade. Contractors who work along with their workers must also fill out a Worker Registration.
Worker Registration Application: Trade Activities [ Form: #JPO-004 ]
This form is used for workers who have experience in certain aspects of a voluntary trade and want to work in Quebec with their Ontario employer but who do not hold a certificate for that trade.
