The following forms are available on our website. Make sure you read the information about completing and submitting forms carefully.
To avoid using outdated versions, we recommend that you do not permanently download any form to your computer.

Notice of Project [Form 0175]
- According to section 6 of the Regulation for Construction Projects (O. Reg. 213/91) constructors are required to notify the Ministry of Labour before construction begins of any project meeting any of the requirements applicable to this section (for example, having a total expected cost of more than $50,000).
- The original signed form must be submitted to your local Ministry of Labour office. It can submitted by fax but the original signed form must still be sent to the local office.

- your local ServiceOntario Centre.
Registration of Constructors and Employers Engaged in Construction [Form 1000]
According to Section 5 of the Regulation for Construction Projects (O. Reg. 213/91), before beginning work at a project every constructor and employer engaged in construction has to complete an approved registration form.
This form does not have to be submitted to the Ministry of Labour, but it must be at the project while the employer is working there.

Window Cleaning / Asbestos Removal / Trench Work Notification
According to the Window Cleaning Regulation (Reg. 859), the Regulation respecting Asbestos on Construction Projects and in Buildings and Repair Operations (O. Reg. 278/05), and the Regulation for Construction Projects (O. Reg. 213/91), employers are required to notify the Ministry of Labour of window cleaning operations, asbestos removal operations, and trench work where a worker may enter a trench which is more than 1.2 metres (4 feet) deep.
Employers can use the electronic Notification for Window Cleaning, Asbestos Removal or Trenching form to register their project or notify the nearest Ministry of Labour office in person, by phone or by fax.
