Form: Employment Standards Claim
If you believe your rights have been violated you may use the Employee Self Help Kit from the Ministry of Labour to resolve the matter with your employer. The Employee Self Help Kit has a letter that you can send to your employer.
You may also file a claim with the Ministry of Labour. Claim forms are available online below as well as at ServiceOntario Centres.
Filing a claim is free.
It is against the law for your employer to punish you for filing a claim.
If you file a claim, Ministry of Labour staff will investigate your claim. Claims are investigated as quickly as possible. The time it takes to complete the process varies.

Making it Easier
Follow these four steps:
- Read the Important Employment Standards Claim Information located at the beginning of the claim form
- Fill out the claim form
- Send your claim form to the Ministry of Labour and receive your claim number
- It is recommended that you file your claim online below. You will receive your claim number immediately.
- You may also file your claim:
- In person at a ServiceOntario Centre (1-800-267-8097). Use PDF version below. You will receive a letter, with your claim number, in the mail.
- By mail to the Provincial Claims Centre, Ministry of Labour, 70 Foster Drive, Suite 480, Roberta Bondar Place, Sault Ste. Marie, ON, P6A 6V4. Use PDF version below. You will receive a letter, with your claim number, in the mail.
- By fax: 1-888-252-4684. Use PDF version below. You will receive a letter, with your claim number, in the mail.
- Please only file your claim once. For example, if you have filed your claim online, please do not send another copy of your claim form to the Ministry of Labour.
- Send copies of your documents to the Ministry of Labour
